What to Bring
Copies of the following documents are needed during the admission process:
- Medicare Card - Part A & B
- Medicare Card - Part D
- Insurance Cards
- Social Security Card
- Durable Power of Attorney / Guardianship Document
- Advance Directives / Living Will
- Long Term Care Insurance Information
Guests will be making Maplewood their home. The necessary furnishings will vary with the size of the apartment. Maplewood has furnishings available for short term use. All apartments are phone and cable ready; activating the phone and cable service is up to each guest and/or family.
